ACCREDITATION CERTIFICATE

This is to certify that:

Amity Institute, dr Jacek Morawski, Warsaw, Poland
has been audited and found to be in substantial compliance with established standards and practices for Aggression Replacement Training (ART) Training Centers.
Approval is hereby granted to operate as a G&G Consultants, LLC Accredited Professional Aggression Replacement Training (ART) Training Center providing the rules and conditions as stipulated in G&G Consultants Standards and Practices for Accredited Professional ART Training Centers relating to accreditation are observed at all times.

ACCREDITATION SCOPE:

• Amity Institute may provide ART Training courses for 1) trainers (facilitators) who wish to implement ART with groups; 2) Training of Trainers' courses for those who have met such requirements; 3) and develop materials to support such efforts in consultation with and permission by Dr. Barry Glick (ART co-developer and author).

• Amity Institute may certify group trainers (facilitators) who wish to implement ART with groups after they complete the necessary and established requirements.

• Amity Institute may certify Trainers to Train group trainers (facilitators) after they meet and complete the necessary and established requirements for Trainers of Trainers.

• Amity Institute may also accredit training institutions throughout Poland to offer Aggression Replacement Training (ART) programs, as well as train others to deliver such programs, according to established standards and requirements.

ACCREDITATION FEES:

Accreditation fees are established under a separate document agreed and executed by all parties.
This accreditation is valid through Midnight 31 December 2004, and is renewable, without change, unless so authorized by G & G Consultants, LLC.
